This article has become extremely busy in the last couple months — about 4,000 readers per day, as described here — so I am really polishing it and making sure that it’s the best possible answer to people’s fears about back pain.

Of course, you know what it feels like to have a tight muscle. But tight hips aren't just uncomfortable—they can lead to all sorts of other aches and pains, especially in the lower back. "People focus on the hips and say their hips are tight, but we don't always think about the fact that the lower back connects to our legs at the hip," Charlee Atkins, C.S.C.S., instructor at Soul Annex in New York City and creator of Le Stretch class, tells SELF. Tight hip flexors make it harder for your pelvis to rotate properly, which can cause your lower back to overcompensate, "and this can be a setup for lower-back injury," Teo Mendez, M.D., an orthopedic surgeon at NY Orthopedics who focuses on operative and non-operative management of sports-related injuries, musculoskeletal injuries, and arthritis, tells SELF.
At the very least, the tension and/or spasm in muscles that cross over the hip and attach onto the pelvis can contribute to imbalance, in terms of how strong and flexible each muscle group is in relation to the others. But muscle imbalance in the hips and the spine may make for pain, limitation and/or posture problems. It can also increase the healing challenge put to you by an existing injury or condition, for example, scoliosis.
Subacute low back pain. Lasting between 6 weeks and 3 months, this type of pain is usually mechanical in nature (such as a muscle strain or joint pain) but is prolonged. At this point, a medical workup may be considered, and is advisable if the pain is severe and limits one’s ability to participate in activities of daily living, sleeping, and working.
Pain along the inside of the hip may be due to tendinitis or strain of the adductor muscles. Adductors (or inner thigh muscles) pull the leg inward as it is moving forward—the faster the movement, the greater the degree of adduction. Since footprints of a runner are almost single file as opposed to the side-by-side footprints of a walker, there is some degree of adduction occurring during running.
Backpack overload in children: Low back pain unrelated to injury or other known cause is unusual in pre-teen children. However, a backpack overloaded with schoolbooks and supplies can strain the back and cause muscle fatigue. The American Academy of Orthopaedic Surgeons recommends that a child’s backpack should weigh no more than 15 to 20 percent of the child’s body weight.
In the elderly, atherosclerosis can cause weakening of the wall of the large arterial blood vessel (aorta) in the abdomen. This weakening can lead to a bulging (aneurysm) of the aorta wall. While most aneurysms cause no symptoms, some cause a pulsating low back pain. Aneurysms of certain size, especially when enlarging over time, can require surgical repair with a grafting procedure to repair the abnormal portion of the artery.

Spinal fusion is used to strengthen the spine and prevent painful movements in people with degenerative disc disease or spondylolisthesis (following laminectomy). The spinal disc between two or more vertebrae is removed and the adjacent vertebrae are “fused” by bone grafts and/or metal devices secured by screws. The fusion can be performed through the abdomen, a procedure known as an anterior lumbar interbody fusion, or through the back, called posterior fusion. Spinal fusion may result in some loss of flexibility in the spine and requires a long recovery period to allow the bone grafts to grow and fuse the vertebrae together. Spinal fusion has been associated with an acceleration of disc degeneration at adjacent levels of the spine.
Kidneys — The kidneys are a matched pair. One painful kidney can cause back pain on one side or the other. Kidney pain can feel like back pain, and may occur on only one side. It is usually quite lateral, and just barely low enough to qualify as “low” back pain. However, when kidney stones descend through the ureters, they can cause (terrible) pain in the low back. Kidney stone pain is often so severe and develops so rapidly that it isn’t mistaken for a back pain problem.
Injury to the bones and joints: Fractures (breakage of bone) of the lumbar spine and sacrum bone most commonly affect elderly people with osteoporosis, especially those who have taken long-term cortisone medication. For these individuals, occasionally even minimal stresses on the spine (such as bending to tie shoes) can lead to bone fracture. In this setting, the vertebra can collapse (vertebral compression fracture). The fracture causes an immediate onset of severe localized pain that can radiate around the waist in a band-like fashion and is made intensely worse with body motions. This pain generally does not radiate down the lower extremities. Vertebral fractures in younger patients occur only after severe trauma, such as from motor-vehicle accidents or a convulsive seizure.
Hip flexors. These hardworking muscles are crucial in foundational movements such as sitting, standing, walking and running — they act as a bridge connecting your torso to your lower body. Some muscles in this group can be notoriously weak or tight and those of you who have ever had issues with this part of your body will know the uncomfortable pain of either all too well.  There’s a lot of debate in the world of sports science over how much you should strengthen and stretch your hip flexors — we’ll explain.

En español | You probably know someone who’s traded a worn-out hip bone for ceramic or cobalt chrome. Some 370,000 Americans undergo hip-replacement surgery each year (the average age for this is 65). But the operation isn’t a cure-all: At least 1 in 10 hip-replacement recipients will need a second procedure to repair a dislocation, mechanical failure or infection. And hip pain, with or without surgery, can be a struggle. Chronic hip pain was a factor in the accidental opioid overdoses that claimed the lives of musicians Tom Petty, 66, and Prince, 57.
A recent study from Florida Atlantic University found that doing 45 minutes of chair yoga twice per week reduced pain and improved overall quality of life in older adults with osteoarthritis. Chair yoga is practiced sitting in a chair or standing while holding the chair for support, so it’s a great option for beginners or anyone dealing with an injury or balance problems.
The bony lumbar spine is designed so that vertebrae "stacked" together can provide a movable support structure while also protecting the spinal cord from injury. The spinal cord is composed of nervous tissue that extends down the spinal column from the brain. Each vertebra has a spinous process, a bony prominence behind the spinal cord, which shields the cord's nervous tissue from impact trauma. Vertebrae also have a strong bony "body" (vertebral body) in front of the spinal cord to provide a platform suitable for weight bearing of all tissues above the buttocks. The lumbar vertebrae stack immediately atop the sacrum bone that is situated in between the buttocks. On each side, the sacrum meets the iliac bone of the pelvis to form the sacroiliac joints of the buttocks.
If all you’ve been doing for your lower body lately is the occasional lunge, it’s time to mix up your routine. “Our bodies move in different directions, not in one direction, so it’s important to mimic that during cross-training,” Whitesides says. That’s why instead of relying on a pattern of “lunge-squat-repeat” — you might want to throw some plié squats, or leg lifts into the mix.

Exercise appears to be useful for preventing low back pain.[47] Exercise is also probably effective in preventing recurrences in those with pain that has lasted more than six weeks.[1][48] Medium-firm mattresses are more beneficial for chronic pain than firm mattresses.[49] There is little to no evidence that back belts are any more helpful in preventing low back pain than education about proper lifting techniques.[47][50] Shoe insoles do not help prevent low back pain.[47][51]
Kneel with a wall or pillar behind you, knees hips-width apart and toes touching the wall. Arch your back to lean back while keeping your hips stacked over your knees. Take your arms overhead and touch your palms into the wall behind you. This bend does not need to be extremely deep to feel a great stretch in the hips and strength in the lower back.
Apply the above concept to your hips. When you sit, your hips are in a "flexed" position. Therefore, the muscles that flex your hips are in a shortened state. You probably spend at least a third of your day sitting down. Think about how much time those hip flexor muscles stay shortened. A lot. Over time, they become tighter and tighter until you look like the old man in the picture. So unless you want to look like that, perform the stretches shown below.
When a muscle contracts, it shortens. Take the biceps for example. Without getting too technical, the biceps are attached at the forearm and shoulder. When your biceps contract, they shorten and bring those two points closer together. When you rest, the muscle returns to its normal length, and the two points move farther away. Constantly contracting your biceps over a long period of time would cause them to get shorter, even at rest.

Athletes are at greater risk of sustaining a lumber spine injury due to physical activity. Whether the sport is skiing, basketball, football, gymnastics, soccer, running, golf, or tennis-the spine undergoes a lot of stress, absorption of pressure, twisting, turning, and even bodily impact. This strenuous activity puts stress on the back that can cause injury to even the finest and most fit athletes.

Strengthening exercises, beyond general daily activities, are not advised for acute low back pain, but may be an effective way to speed recovery from chronic or subacute low back pain. Maintaining and building muscle strength is particularly important for persons with skeletal irregularities. Health care providers can provide a list of beneficial exercises that will help improve coordination and develop proper posture and muscle balance. Evidence supports short- and long-term benefits of yoga to ease chronic low back pain.
There are cases of low back pain that have alarming causes, but it’s rare. Once in a while back pain is a warning sign of cancer, autoimmune disease, infection, or a handful of other scary culprits.7 Over the age of 55, about one in twenty cases turns out to be a fracture, and one in a hundred is more ominous.8 The further you are from 55, the better your odds.

I’m not endorsing or saying these stretches are a miracle cure, but I definitely think they’ve helped. I’ve even started to add a few gentle yoga poses and pilates moves which I could so easily do, back in the day, when I was 63 kilos and participated in the greatest oxymoron ever named … a thing called the Fun Run. If I’m not careful, and I keep stretching away each day, I could end up with a 6 pack and back to my 5′ 7″ instead of 5′ 5″.
A few cancers in their early stages can be hard to tell apart from ordinary back pain — a bone cancer in the vertebrae, for instance — and these create a frustrating diagnostic problem. They are too rare for doctors to inflict cancer testing on every low back pain patient “just in case.” And yet the possibility cannot be dismissed, either! It’s an unsolveable problem.
How to do it: Loop a resistance band around your ankles and lie on your right side with your right arm extended on the floor, left hand in front of your body for support. Brace your abs in tight, bring your bottom leg slightly in front of your top leg (let the inside of your top foot rest on the ground), and lift your leg up to the ceiling, keeping your hips stacked. Maintain tension on the band at all times and pulse your bottom leg up and down quickly 20 times. Repeat on the other side.
Arthritis: The spondyloarthropathies are inflammatory types of arthritis that can affect the lower back and sacroiliac joints. Examples of spondyloarthropathies include reactive arthritis (Reiter's disease), ankylosing spondylitis, psoriatic arthritis, and the arthritis of inflammatory bowel disease. Each of these diseases can lead to low back pain and stiffness, which is typically worse in the morning. These conditions usually begin in the second and third decades of life. They are treated with medications directed toward decreasing the inflammation. Newer biologic medications have been greatly successful in both quieting the disease and stopping its progression.

Health care professionals diagnose hip pain with a history and physical examination. Physical examination maneuvers, such as internally and externally rotating the hip, can be used to detect pain-aggravating positions. Tenderness can be elicited by palpating over inflamed areas. Straight leg raising can detect signs of sciatica. A health care professional may use imaging studies, including X-rays, CT scans, and MRI scans, to further define the causes of hip pain. Sometimes, nuclear medicine bone scans are used to image inflamed or fractured bone.
Imaging is indicated when there are red flags, ongoing neurological symptoms that do not resolve, or ongoing or worsening pain.[5] In particular, early use of imaging (either MRI or CT) is recommended for suspected cancer, infection, or cauda equina syndrome.[5] MRI is slightly better than CT for identifying disc disease; the two technologies are equally useful for diagnosing spinal stenosis.[5] Only a few physical diagnostic tests are helpful.[5] The straight leg raise test is almost always positive in those with disc herniation.[5] Lumbar provocative discography may be useful to identify a specific disc causing pain in those with chronic high levels of low back pain.[41] Similarly, therapeutic procedures such as nerve blocks can be used to determine a specific source of pain.[5] Some evidence supports the use of facet joint injections, transforminal epidural injections and sacroilliac injections as diagnostic tests.[5] Most other physical tests, such as evaluating for scoliosis, muscle weakness or wasting, and impaired reflexes, are of little use.[5]
3. Hug it out. Start the supine hip flexor stretch the same as the glute bridge, but keep the right leg relaxed on the floor. Pull shoulder blades down and back to lift hips. Grab the back thigh of the left leg and pull the knee toward the chest. Keep the right leg straight and push its heel into the floor (to feel it in the butt). Hold for 30-45 seconds and switch legs.

How to: Stand tall, feet slightly wider than shoulder-width apart, toes pointed out at 45-degree angles (a). Keep your back straight, knees over toes and your weight in the heels of your feet (b). Engage your glutes and thighs as you lower into a deep squat until thighs parallel to the ground (or as close as you can get them) (c). Powering through your heels, push up to return to starting position (d). Repeat.

This stretch gets at the piriformis muscle of the hip flexor. Sit with both legs extended in front of you. Bend the right knee and place the right foot on the floor. Place your right hand behind you and hook your left elbow on the outside of the right knee. As you twist toward the righthand side, keep your spine straight and breathe deeply. Switch sides after about 20 seconds.

Walking is an easy way to strengthen your bones and muscles, burn calories, and lift your mood. If you don't currently walk for exercise, try walking for five to 10 minutes every day. Gradually build up to 30 minutes a day. It's easy to sneak walking into your day. Take up golf, window-shop at a large mall, or visit a walking-only destination, such as a museum or botanical garden.
Epidural steroid injections are most commonly used in situations of radicular pain, which is a radiating pain that is transmitted away from the spine by an irritated spinal nerve. Irritation of a spinal nerve in the low back (lumbar radiculopathy) causes pain that goes down the leg. Epidural injections are also used to treat nerve compression in the neck (cervical spine), referred to as cervical radiculopathy, which causes pain.

The big idea of classification-based cognitive functional therapy (CB-CFT or just CFT) is that most back pain has nothing to do with scary spinal problems and so the cycle of pain and disability can be broken by easing patient fears and anxieties. For this study, CFT was tried with 62 patients and compared to 59 who were treated with manual therapy and exercise. The CFT group did better: a 13-point boost on a 100-point disability scale, and 3 points on a 10-point pain scale. As the authors put it for BodyInMind.org, “Disabling back pain can change for the better with a different narrative and coping strategies.” These results aren’t proof that the confidence cure works, but they are promising.

Irritation of a spinal nerve in the low back (lumbar radiculopathy) causes pain that goes down the leg. Epidural injections are also used to treat nerve compression in the neck (cervical spine), referred to as cervical radiculopathy, which causes pain.
Brace your core and bend your knees to lower down into a split squat. Your left knee should ideally form a 90-degree angle so that your thigh is parallel to the ground, and your right knee is hovering above the floor. (Quick position check: your left foot should be stepped out far enough that you can do this without letting your left knee go past your left toes—if you can't, hop your left foot out a bit farther away from the bench.)
"Lower back pain is the most common musculoskeletal ailment in the U.S., and can often be mitigated by strengthening the core musculature," Blake Dircksen, D.P.T., C.S.C.S., a physical therapist at Bespoke Treatments New York, tells SELF. "The 'core' is a cylinder of abdominal and back muscles that wraps around the body like a corset," Dircksen explains. (The glutes are also considered a part of the core, since they connect to the pelvis and ultimately the back and abdominal muscles.) As with any muscles, by strengthening them, you will increase the amount of weight your lower back can comfortably move, which means it will be better equipped to handle the same stress from your workouts and everyday life without getting as achey.

But that’s what our bodies would be like without our hip flexor muscles. Never heard of ‘em? It’s about time we share why they’re so important, how your desk job might be making them weaker (ah!), and the best ways to stretch them out.

There is a significant overlap of nerve supply to many of the discs, muscles, ligaments, and other spinal structures, and it can be difficult for the brain to accurately sense which is the cause of the pain. For example, a degenerated or torn lumbar disc can feel the same as a pulled muscle – both creating inflammation and painful muscle spasm in the same area. Muscles and ligaments heal rapidly, while a torn disc may or may not. The time course of pain helps determine the cause.

Massage therapy does not appear to provide much benefit for acute low back pain.[1] A 2015 Cochrane review found that for acute low back pain massage therapy was better than no treatment for pain only in the short-term.[89] There was no effect for improving function.[89] For chronic low back pain massage therapy was no better than no treatment for both pain and function, though only in the short-term.[89] The overall quality of the evidence was low and the authors conclude that massage therapy is generally not an effective treatment for low back pain.[89]
