If you’re worried you’re headed toward a surgeon’s office, there might be hope. According to the Arthritis Foundation, the best way to avoid hip replacement surgery is to get active in an exercise program. In a study, people who participated in an exercise program for 12 weeks were 44 percent less likely to need joint-replacement surgery six years later than those who did not exercise.

The big idea of classification-based cognitive functional therapy (CB-CFT or just CFT) is that most back pain has nothing to do with scary spinal problems and so the cycle of pain and disability can be broken by easing patient fears and anxieties. For this study, CFT was tried with 62 patients and compared to 59 who were treated with manual therapy and exercise. The CFT group did better: a 13-point boost on a 100-point disability scale, and 3 points on a 10-point pain scale. As the authors put it for BodyInMind.org, “Disabling back pain can change for the better with a different narrative and coping strategies.” These results aren’t proof that the confidence cure works, but they are promising.

Iliopsoas syndrome, which is also called psoas syndrome or iliopsoas tendonitis, occurs when the iliopsoas muscles are injured. Lower back pain is the most common symptom; however, pain can also occur in the hip, thigh, or leg. The iliopsoas bursa, which is a fluid-filled sac located on the inside of the hip that reduces rubbing and friction, is also likely to become inflamed due to the proximity of the two structures. When this happens, the inflamed bursae will make it difficult to move.

The pain of back pain almost always makes it seem worse than it is. The most worrisome causes of back pain rarely cause severe pain, and many common problems (like slipped discs) are usually much less serious than people fear. Only about 1% of back pain is ominous, and even then it’s often still treatable. Most of the 1% are due to cancer, autoimmune disease, or spinal cord damage.
How to: Stand tall, feet slightly wider than shoulder-width apart, toes pointed out at 45-degree angles (a). Keep your back straight, knees over toes and your weight in the heels of your feet (b). Engage your glutes and thighs as you lower into a deep squat until thighs parallel to the ground (or as close as you can get them) (c). Powering through your heels, push up to return to starting position (d). Repeat.
If low back pain occurs after a recent injury — such as a car accident, a fall or sports injury — call your primary-care physician immediately. Lumbar strain (acute, chronic): A lumbar strain is a stretch injury to the ligaments, tendons, and/or muscles of the low back. The stretching incident results in microscopic tears of varying degrees in these tissues. Lumbar strain is considered one of the most common causes of low back pain. The injury can occur because of overuse, improper use, or trauma. Soft-tissue injury is commonly classified as "acute" if it has been present for days to weeks. If the strain lasts longer than three months, it is referred to as "chronic." Lumbar strain most often occurs in people in their 40s, but it can happen at any age. The condition is characterized by localized discomfort in the low back area with onset after an event that mechanically stressed the lumbar tissues. The severity of the injury ranges from mild to severe, depending on the degree of strain and resulting spasm of the muscles of the low back. The diagnosis of lumbar strain is based on the history of injury, the location of the pain, and exclusion of nervous system injury. Usually, X-ray testing is only helpful to exclude bone abnormalities.

For example, your quadriceps muscles are a group of four that are located at the front of the thigh; one of the group members, the rectus femoris flexes the hip, which brings your lower extremity (thigh, lower leg, and foot) forward, in front of you. Hip hikers (also known as the pelvic drop) are great exercises to get your gluteal muscles working in a weight bearing position. To do the exercise, stand sideways with one foot on a step and the other hanging off. Keeping both knees straight, lower down your pelvis on one side so your foot moves toward the floor. Both knees should remain straight; the motion should come from your hip joint. Once your pelvis is lowered down, slowly raise it back up to the starting position. Repeat the exercise for 10 repetitions.
Nonsteroidal anti-inflammatory drugs (NSAIDS) relieve pain and inflammation and include OTC formulations (ibuprofen, ketoprofen, and naproxen sodium). Several others, including a type of NSAID called COX-2 inhibitors, are available only by prescription. You can strain or tear one or more of your hip flexors when you make sudden movements such as changing directions while running or kicking. Sports and athletic activities where this is likely to occur include running, football, soccer, martial arts, dancing, and hockey. In everyday life, you can strain a hip flexor when you slip and fall, for example.
Pain on the outside of the hip is most commonly due to greater trochanteric bursitis. The greater trochanter is the protrusion where the thigh bone juts outward at the base of the neck (which connects the ball to the femur and is the site of hip stress fractures). A lubricating sac (or bursa) lies over the boney protrusion so that the surrounding muscles do not rub directly on the bone. The top region of the iliotibial (IT) band, known as the tensor fascia lata, is commonly involved in greater trochanteric bursitis.

Vertebroplasty and kyphoplasty are minimally invasive treatments to repair compression fractures of the vertebrae caused by osteoporosis. Vertebroplasty uses three-dimensional imaging to assist in guiding a fine needle through the skin into the vertebral body, the largest part of the vertebrae. A glue-like bone cement is then injected into the vertebral body space, which quickly hardens to stabilize and strengthen the bone and provide pain relief.
